Hot flashes that arrive in your early forties. Sleep that never quite feels restful. Periods that change without warning, anxiety you do not recognize, and labs that come back “normal.” If this sounds familiar, you may be in perimenopause, and you deserve care from someone who understands it.
What is perimenopause?
Perimenopause is the transition before menopause, when hormone levels begin to fluctuate. According to Cleveland Clinic, it usually begins in your mid-forties but can start as early as your mid-thirties, and it lasts about four years on average, sometimes up to eight. You reach menopause once you have gone 12 months in a row without a period.
Because hormones rise and fall rather than drop steadily, perimenopause symptoms can come and go, and a single blood test often misses what is happening. That is why many women are told everything looks fine when it does not feel fine.
What are the signs of perimenopause?
- Irregular, heavier or lighter periods
- Hot flashes and night sweats
- Trouble falling or staying asleep
- Anxiety, irritability or low mood
- Brain fog and trouble concentrating
- Lower sex drive or vaginal dryness
- Weight changes, especially around the middle
- Joint aches, fatigue and low energy

Is hormone therapy safe during perimenopause?
For most healthy women, yes. The Menopause Society’s 2022 hormone therapy position statement concludes that the benefits outweigh the risks for most healthy women younger than 60 and within 10 years of menopause onset. Your provider reviews your personal and family history first, because hormone therapy is not right for everyone. Learn more in Is HRT Safe? Frequently asked questions about perimenopause care
What is the best age to see someone for perimenopause?
There is no minimum age. Perimenopause usually begins in the mid-forties but can start in the mid-thirties. If your periods, sleep, mood or energy have changed and you are over 35, it is reasonable to see a provider trained in menopause care.
Can a blood test diagnose perimenopause?
Not on its own. Hormone levels fluctuate during perimenopause, so a single test can look normal. Diagnosis is based mainly on your age, symptoms and cycle changes, with labs used to rule out other causes such as thyroid problems or low iron.
Can I start HRT during perimenopause?
Yes, many women can. Hormone therapy can be started during perimenopause to treat symptoms such as hot flashes, night sweats and sleep disruption. Your provider will review your health history and choose a dose and delivery method that fits you.
