Founders Circle
For the original 25 founding members. Learn about the Founders Circle.
Founders pricing is available to the original 25 founding members, for a limited time. Once the 25 seats fill or the founders window closes, the offer ends and standard rates apply. The $999 rate is locked in for your first two years.
Your membership is $999 per year for your first two years. Pricing for subsequent years is subject to change and will be communicated in writing no less than 30 days before your renewal date. You may cancel at any time before your renewal date — but cancelling permanently forfeits the founders rate, and re-enrollment is available at standard membership pricing.
No multi-year commitment. Founders membership is billed annually — $999/year for your first two years, with pricing for later years communicated in writing at least 30 days before renewal. You can cancel at any time before your renewal date by contacting us directly.
We currently practice in Arizona, California, Florida, Georgia, Massachusetts, Nevada, and Texas. You'll need to be a resident of one of these states to join. If you're outside these states, join our general waitlist. We expand as licensure allows.
Not at the moment. We're cash-pay only. That's how we can offer long-form visits, direct messaging, and same-day access without the administrative overhead. We don't currently provide reimbursement documentation for out-of-network claims.
Included: the initial visit with Dr. Chorney, direct M–F messaging, same / next-day appointments, priority booking ahead of standard Concierge members, prescription refill management, and a 20% standing discount on coaching, nutrition, and non-insured tests. Not included: medications themselves (we'll route refills to the most affordable pharmacy), specialty labs and tests (discounted 20%), and care from outside providers.

Yes, anytime — just contact Amsara Health directly. Cancellations take effect on the date the request is processed, and benefits cease at that point. Annual fees are non-refundable, but any unused portion of your term converts to à la carte service credit, valid for 12 months toward visits, refill call-ins, lab reviews, clinical messages, or coaching. If you're on active therapy (HRT, GLP-1, or other long-term prescriptions), our clinical team coordinates a safe transition and a final 30-day refill where appropriate. Note that cancelling permanently forfeits the founders rate. Full details are on our membership terms page.
Dr. Chorney is a doctorally-trained nurse practitioner with focus areas in menopause, cardiology, genomics, and sports nutrition. She cares for physicians, CEOs, and executive leaders, and is the clinician other doctors trust with their patients and family members. Her practice is built on rigor, access, and the kind of attention modern medicine rarely affords.
It's built for perimenopause, menopause, and the years after, but Dr. Chorney's training spans cardiology, genomics, and sports nutrition. If you're navigating midlife and want a single clinician who sees the whole picture, this is the relationship.
Standard Membership

Every Amsara membership covers a long-form initial visit, direct messaging with your provider, same and next-day appointments when something shifts, prescription refill management, and member-rate access to specialty labs and tests. The relationship is the product, not a series of one-off visits.
Because midlife care isn't a single visit. Hormones, sleep, mood, energy, and metabolism change over time, and the work of staying well requires continuity. Members get direct access to their provider between visits, which is what makes the model actually work.
Membership is $199 per month or $1,999 per year (annual saves $389, about 16%). The price covers everything listed under What's Included. Medication itself is not included, and specialty labs are offered at member rates.

Your provider can prescribe directly when it's appropriate. We work with mail-order and local pharmacies and route refills to the most affordable option. Medication cost itself is not included in membership.
Yes. We'll publish full cancellation terms when public enrollment opens. The short version: no contracts that prevent you from leaving, and clear refund policies.
